How to Complete an Assessment on Behalf of a Patient

Overview
If a patient is unable to complete their assessment digitally, you can complete it for them during a face-to-face or telephone appointment.
Steps to Complete an Assessment on Behalf of a Patient:
- Locate the patient:
- Use the Search bar at the top of the portal to find the patient by name or identifier,
- Or browse one of the patient folders (Needs Attention, In Progress, etc.).
2. Open the patient page and go to the “Assessment” tab.
3. Click the “Create clinician link” button.
- A pop-up will ask you to confirm that you want to create a clinician access link to the patient's assessment.
- Note: Creating a clinician link invalidates any existing patient links (so the patient won’t be able to access their assessment during this time).
4. Click the link in the pop-up window to open the patient’s assessment.
5. You will be prompted to enter the patient’s date of birth for verification (you can find this on the patient page).
6. Complete all the required fields in the assessment form on behalf of the patient.
7. Submit the questionnaire once completed.
Additional Information
- The patient details and responses will be automatically updated and saved in the MyPreOp Base portal.
This process ensures accurate documentation and supports patients who cannot complete the assessment digitally on their own.
