### **Steps to Complete an Assessment on Behalf of a Patient:**

- Use the **Search** bar at the top of the portal to find the patient by name or identifier,
- Or browse one of the patient folders (Needs Attention, In Progress, etc.).

3\. Click the “Create clinician link” button.

- A pop-up will ask you to confirm that you want to create a clinician access link to the patient's assessment.
- **Note:** Creating a clinician link **invalidates any existing patient links** (so the patient won’t be able to access their assessment during this time).

4\. Click the link in the pop-up window to open the patient’s assessment.
5\. You will be prompted to enter the **patient’s date of birth** for verification (you can find this on the patient page).
6\. Complete all the **required fields** in the assessment form on behalf of the patient.
7\. Submit the questionnaire once completed.

**Additional Information**

- The patient details and responses will be automatically updated and saved in the MyPreOp Base portal.

This process ensures accurate documentation and supports patients who cannot complete the assessment digitally on their own.
